As you may have noticed here, i am not a fan, at all, of naming someone – giving them a label based upon appearances and reactions to meds. I know that’s the rage now, and know that a mood stabilizer might work but that does not solve your underlying problem.
I strongly suggest that you have a moving therapeutic window, and subsequent narrowing of the therapeutic window, and a refractory response to meds based on completely other issues than an underlying bipolar illness.
Type metabolism in the Search here and spend some time with those posts and you will get what I am talking about… and for the low, low price of 10 bucks you can get phenomenal details in my *Rules* book!

Good evening Dr. P,
I’ve been away for a long while and have done ok. Most recently (and for several months now) I’ve been taking adderall 30 mg twice a day, effexor 150 once, and lorazepam 1 as needed in the evening.
The adderall by far is the most effective. It seems that the effexor sort of helps some of the similar things, but not much. The lorazepam I didn’t notice any difference.
So today, I had my appointment with my psychologist and we talked, and questions,talked, etc..And it’s come down to the two things the most right now that struggle to manage is intrusive thoughts (some of which are very scary), the subsequent anxiety, and all my doubts about my diagnoses (and I gave her a list of reasons why).
I’ve been on every antidepressant and none of them do anything helpful. Effexor doesn’t hurt,so that’s why were still on it. Was originally supposed to go up to 225, but right before I was supposed to do that I had some issues at work that meant it would be better to not add a new variable into the mix.
So thoughts I guess…may be helpful. She said I came up with a moderate probability of bipolar and Iv’e never taken any mood stabilizers before. So far adderall and effexor has been the best combo, except the effexor hasn’t really addressed what it was supposed to yet.
Any ideas?
